Your SMP journey begins well before your first session — it begins with a free consultation. This is one of the most important steps in the process, and we take it seriously at every one of our clinics.

  • Assessment of your hair loss — your practitioner evaluates the degree and pattern of your hair loss, your scalp condition, any existing scarring, and your skin tone

3. How to Prepare for Your First Session

Arriving well-prepared makes a real difference to how your skin responds to treatment, how comfortable the session is, and how well the pigment settles during healing.

In the days before your appointment

  • Stay well hydrated. Well-hydrated skin accepts pigment more evenly and heals more efficiently. Drink plenty of water in the 48 hours before your session.
  • Avoid alcohol for at least 24 hours beforehand — ideally 48. Alcohol thins the blood, which can cause increased bleeding and uneven pigment uptake.
  • Skip the gym the day before. Elevated circulation and sweating in the hours prior to treatment can affect the session.
  • Protect your scalp from sunburn. Sunburned or peeling skin cannot be treated safely. Avoid prolonged direct sun on your scalp for at least a week before your appointment.
  • Pause harsh scalp products. Avoid strong exfoliants, chemical treatments, or direct application of minoxidil to the scalp for a few days prior.
  • Don’t take blood thinners (including aspirin, fish oil, or vitamin E supplements) unless medically necessary — check with your GP first and inform your practitioner.

On the day of your appointment

  • Wash your scalp with your usual shampoo that morning
  • Arrive with a clean scalp — free of wax, pomade, or styling products
  • Eat a proper meal before your session — low blood sugar can cause lightheadedness during longer appointments
  • Wear a dark, loose-fitting top that is easy to remove without disturbing your head
  • Allow plenty of time — first sessions typically run between two and four hours
  • Bring headphones if you’d like to listen to music or a podcast during the session

Important: If you are taking prescription blood-thinning medications such as warfarin or clopidogrel, please discuss this with your GP before your appointment and inform your practitioner at consultation. Do not cease prescription medication without medical advice.

4. What Happens on the Day

When you arrive for your first session, your practitioner will begin by reviewing the plan established at your consultation. If this is your first visit and you haven’t had a separate consultation, this assessment and design process happens at the start of your appointment.

You’ll be settled into a comfortable, reclined treatment chair in a clean, professional clinic environment. The atmosphere is calm and unhurried — most clients find the experience far more relaxed than they anticipated.

Your practitioner will walk you through exactly what they’re going to do before they start, answer any last-minute questions, and ensure you’re completely comfortable before the treatment begins.

5. What Happens in the Chair

Step 1: Hairline mapping and design confirmation

Before any pigment is applied, your practitioner will map out the hairline and treatment zones using a skin-safe pencil. You’ll view this in a mirror together — and nothing proceeds until you’re completely happy with what you see. This is your opportunity to ask for adjustments. It’s a collaborative process.

Step 2: Pigment preparation

Your customised pigment blend is prepared. SMP uses pigments that are specifically formulated for scalp work — not standard tattoo inks. Quality SMP pigments are designed to remain colour-stable over time, without the blue, green, or grey shift that occurs when inappropriate pigments are used.

Step 3: Treatment

Using a fine needle — and either a rotary device or hand technique depending on the area — individual pigment impressions are placed methodically across the scalp. Your practitioner works in layered passes, checking density, symmetry, and consistency throughout. A good SMP practitioner is not rushing — they are building a result that needs to look natural from every angle, in every lighting condition.

Step 4: Review and close

At the end of the session, your practitioner will review the work with you in a mirror and take photographs to document the result at this stage. You’ll be given your full aftercare instructions — in writing — before you leave.

What you’ll see after Session 1: The pigment will appear slightly darker and bolder than the final result. This is completely normal — the colour softens and settles significantly during the healing process. Your practitioner will explain exactly what to expect as you heal.

6. Does SMP Hurt?

This is one of the most common questions we receive — and the honest answer is: for most people, it is very manageable.

The sensation is most commonly described as a mild scratching or light stinging — similar to light tattooing. Most clients are surprised by how tolerable it is. Many chat with their practitioner throughout the session, listen to music, or simply relax.

That said, certain areas of the scalp are more sensitive than others:

  • The temples — typically the most sensitive area, particularly near the hairline edges
  • The occipital area (back of the head, near the bone) — can be more sensitive than the crown
  • The crown and top of the scalp — generally well-tolerated by most clients

Topical numbing cream is available and can be applied to more sensitive areas on request. If you have particular concerns about discomfort, let your practitioner know at the start of the session.

Tip: Arriving well-rested and having eaten a proper meal beforehand makes a genuine difference to how comfortable your session feels — particularly for longer first appointments.

7. How Long Does a First Session Take?

A first SMP session typically takes between two and four hours, depending on the extent of the treatment area.

  • Smaller treatments (minor recession, scar camouflage, hairline band) — typically 2–3 hours
  • Full shaven-look treatments (Stage 3–4 hair loss) — typically 3 hours
  • Density treatments for women — typically 2–3 hours depending on the area of thinning

Subsequent sessions are generally shorter than the first, as they are building and refining an existing base rather than starting from scratch. We never rush sessions — the time taken is whatever is needed to do the work properly.

8. Why SMP Takes More Than One Session

All SMP treatments at Scalp Micropigmentation Australia are completed across a minimum of three sessions — and this is by design, not a limitation.

Why multiple sessions produce better results

  • Skin retention varies. After the first session, the scalp sheds a portion of the surface pigment as it heals. The second session builds on what has retained, filling any gaps and adding depth.
  • Layering creates realism. Real hair follicles have subtle variations in shade, size, and density. Multiple passes allow your practitioner to create a layered, three-dimensional result that looks genuinely natural rather than flat or uniform.
  • Refinement is part of the process. Session one is a base layer, session two and three creates much more density to match your own exsisting stubble. Soft hairlines become complete by session three and look basic just from layer 1.

9. Immediately After Your Session

When you leave the clinic after your first session, your scalp will look noticeably treated. Here’s what to expect in the first few hours:

  • Slight redness — the scalp will be mildly red from the needling, similar to mild sunburn. This typically subsides within a few hours. There is no pain after the session. Most clients can return to work if the enviroment they work in is clean and won’t pose a threat of infection.
  • Darker-than-final pigment — the colour will look bolder and sharper than your finished result. This is normal. The pigment softens significantly as healing progresses.
  • Mild tenderness — the scalp may feel slightly tender to the touch, similar to mild sunburn sensitivity. This passes quickly for most people.
  • No bandaging required — you simply walk out and go about your day, with a few important precautions to follow.

Most clients return to work and normal activities the following day without any issue. The treated area is not visibly raw or wounded in the way a surgical procedure would be. 10. Aftercare: The First Two Weeks

How you look after your scalp in the days following treatment directly affects the quality, evenness, and longevity of your result. Please follow these guidelines carefully.

Days 1–5: The protection phase

  • Do not wet the scalp — no washing, swimming, steam rooms, or soaking in the bath for at least five days
  • Avoid sweating — no intense exercise, saunas, hot yoga, or anything that causes significant perspiration
  • Do not touch or pick the treated area — let any surface flaking happen naturally
  • Keep out of direct sun — wear a clean hat when outdoors
  • Sleep on a clean pillowcase — avoid friction or pressure on the scalp during sleep

Days 6–7: The settling phase

  • Gently wash the scalp from around day 5 using a mild, fragrance-free cleanser
  • Continue to avoid prolonged sun exposure without protection
  • Apply a thin layer of fragrance-free moisturiser if the scalp feels dry — avoid petroleum-based products
  • Avoid shaving the treated area until fully healed — approximately day 7
  • Do not use exfoliating or active scalp treatments until healing is complete

Important: The most common aftercare mistake is wetting the scalp too soon or exercising heavily in the first few days. Either can cause patchy pigment retention and an uneven result. The two-week healing window is short — protecting it carefully is well worth the temporary inconvenience.

11. What Your Scalp Looks Like During Healing

Understanding the normal stages of healing prevents unnecessary concern and helps you trust the process.

Days 1–3

The pigment looks dark and defined. The scalp may be slightly red and feel mildly tender. This stage often produces the most dramatic-looking result — darker and crisper than the finished treatment will be.

Days 4–7

The scalp begins to flake lightly as the surface layer of skin renews itself. The pigment may appear to lighten or fade significantly during this phase. This is completely normal and expected. Do not be alarmed — this is not the final result. The underlying pigment is settling into the dermis.

Days 7–14

Session two is completed. The surface flaking subsides and the scalp begins to look more settled and natural. The pigment appears softer and more realistic than it did in the first few days.

After day 21

The scalp has healed sufficiently for your second and thrid session and the true colour and depth of the treatment becomes visible. Most clients find the result at this stage already looks significantly better than they expected at day five.

Remember: You will likely go through a phase during healing where the pigment looks lighter or patchy and you feel concerned. Every client goes through this phase. It is a normal part of the healing process, not a sign that something has gone wrong. Your second session is where the depth, definition, and density comes together.

12. Long-Term Care and Touch-Ups

Once your treatment is complete and fully healed, SMP requires very little ongoing maintenance — one of its greatest practical advantages over other hair loss solutions.

Day-to-day care

  • Apply SPF 30–50 sunscreen to the scalp whenever you are in direct sun. UV exposure is the primary cause of premature pigment fading — consistent sun protection significantly extends the life of your treatment.
  • Moisturise regularly. Keeping the scalp well-hydrated helps maintain skin suppleness and keeps the pigment looking fresh.
  • Avoid prolonged use of harsh exfoliants over the treated area.

Colour refresh sessions

SMP is long-lasting, but not permanent. Over time — typically three to six years — the pigment gradually lightens as the skin naturally renews itself and UV exposure slowly breaks down the pigment molecules. When this happens, a colour refresh session restores depth and definition at a fraction of the cost and time of your original treatment.

Clients who protect their scalp consistently with SPF tend to get closer to the six-year end of that range before a refresh is needed.

15. Frequently Asked Questions

Can I drive myself home after my first SMP session?

Yes. SMP does not involve any anaesthetic, sedation, or procedure that would impair your ability to drive. You are free to drive yourself home or to your next destination directly after your session.

Can I go to work the next day?

Most clients return to office-based or light work the following day without any issue. The scalp will not look raw or bandaged — it simply looks like a very fresh shaved head or newly treated hair. Clients in physically demanding roles or those who need to avoid sweating may wish to take an extra day.

Will people be able to tell I’ve had something done?

In the first day or two, the pigment is at its darkest and may look freshly done to someone who knows what to look for. By the end of the first week, and certainly after full healing, the result looks natural. Most people will simply think you have a shaved head or fuller hair — not that you’ve had a cosmetic procedure.

What if I have a special event coming up?

Ideally, schedule your first session at least three weeks before any significant event — a wedding, formal occasion, or important professional commitment. This gives the scalp time to heal and settle through the natural fading phase before your second session brings the result together.

Can I colour my remaining hair before or after?

Avoid chemical hair colouring on the scalp for at least two weeks after each session. If you regularly colour your hair, time your last colour treatment to be completed at least one week before your SMP session.

What if I am unhappy with the result after my first session?

It is important to assess the result after full healing — not immediately after the session or during the peeling phase. If you have concerns after healing, contact your practitioner before your next session. Minor adjustments can almost always be addressed in subsequent sessions, and your practitioner wants to know about any concerns so they can be addressed properly.

How long should I wait between sessions?

We recommend a minimum of 10 days and typically around two weeks between sessions. This allows the scalp to heal fully and gives you a clear picture of what has retained before the next layer is applied.

I am on minoxidil or finasteride — do I need to stop before my session?

Oral medications such as finasteride or dutasteride do not need to be ceased before SMP. For topical minoxidil, we recommend not applying it directly to the scalp for a few days before your session and pausing use during the first two weeks of healing after each session, as it can irritate healing skin. Discuss this with your practitioner at consultation.
